Purchasers and builders will want to routinely receive information that allows them to assess the program's operation and performance, especially whether it ultimately benefits patients with chronic illnesses. Yet there is at present no standard format for such information, and a purchaser or builder of a self- management support program will find a broad array of possible evaluation measures. To some extent, the choice depends on the program's main goals, but a selection of endpoints may lead to the best understanding of how the program is working. In the following section, . Additional methodological issues are discussed in the next section, . One can examine whether: Program structure matches what was called for in the contract. Coaches are engaging eligible patients and performing the self- management support activities. The mission of the focus in Program Planning and Evaluation is to train students to develop, improve, and critique public heath program plans and evaluations. This focus is designed to assist individuals to become responsible. Policy & Program Planning, Implementation & Evaluation. Planning public health programs. Patients' health- related behaviors have changed. Rates of provider adherence to guidelines have increased. Disease control has improved. Patient health outcomes have improved. Patient satisfaction has improved. Utilization has declined, and patient productivity has improved. Health care costs have declined. Each of these interim and long- term goals is important and provides a possible endpoint for evaluation. They are discussed in more detail below. Tables 3a, 3b, and 3c provide an overview of the endpoints that are examined in the recent literature on existing self- management support programs. Planning and Evaluation Wizard. The NSW Health Planning and. The Community Health Worker Evaluation Tool Kit was developed to assist in evaluating programs Health Workers are involved. The columns in the table parallel the boxes in Figure 1. It can be seen that endpoints all along the chain of effect (following program structure) have been utilized by researchers. Among our interviewees, some use of each category was reported. Neither our literature review nor our interviews identified research on programs that measured endpoints in all categories. Utilization, costs, provider behavior, and disease control endpoints received the greatest emphasis, most likely reflecting considerations of data availability, reliability, and other tradeoffs associated with different data sources. Measures of Program Structure. Commonly, purchasers of external services look for accreditation of a program as a measure of the program's structural soundness. While few studies in the literature used structural measures for program evaluation, a purchaser or developer might want to monitor whether the program has the components and features that were called for in the original plan or contract. One might also question if the features are plausibly capable of supporting the kinds and extent of self- management support activities desired. Are the staff and caseload as expected? Do staff members have the qualifications and training to perform their duties? Can they reasonably be expected to support the kinds of activities envisioned for the intended number of patients? Are procedures and protocols in place to ensure that coaching tasks, as well as education, are performed? Structural measures also may be used to assess whether claims about the program's success based on other measures are plausible. Most structural measures rely on information from the program's management. Patient self- report is another data source to consider. The Disease Management Association of America (DMAA) Participant Satisfaction Survey, for example, asks patients if their program has a toll- free number they can call. Self- management Support Process Measures. Program Evaluation for Public Health Programs: A Self-Study Guide Suggested Citation: U.S. Department of Health and Human Services. We thank the following people from the Evaluation Manual Planning Group for their assistance in. Program planning, implementation and evaluation tools. The GTO approach can be used at any stage of program planning. Planning, Implementing, and Evaluating Health Promotion Programs.Another approach to evaluation is to monitor the performance of the program staff by examining the extent to which they perform the tasks and activities intended; i. Not only are program process measures critical for program supervision and management, they also can tell purchasers and developers if the program is being implemented according to plan. Measures of reach and implementation can help reveal factors that contribute to success or failure and be useful for monitoring of staff performance and program improvement. As shown in Table 3, process measures in the program literature have focused on the reach of the program, processes for assessing patients' self- management skills and needs, education processes, and coaching processes. Measures of . Examples include the percent of the eligible population successfully contacted with an offer of self- management support services, enrollment rates (or opt- in or opt- out rates), completion rates, and drop out rates. As an example, the study of the asthma call center program described earlier reported that 4. An intermediate implementation measure of the program's attempts to engage patients (e. One informant emphasized that it is important for all aspects of evaluation to take into account at the beginning the number of people who are eligible for the program. Different data sources may lead to different definitions of the total population. Programs that rely on claims data will limit the total to individuals who have been diagnosed (or received some specific treatment). Yet, a significant proportion of individuals with some conditions go undiagnosed. If the intent is to capture all people with the condition, as advocated by many proponents of the chronic care model, then health risk assessments or other screening mechanisms may be necessary to identify the total population. Who is being reached is another consideration. It is important that the people reached were, in fact, part of the target population. Another critical issue is whether the people reached are disproportionately those who already are most likely to self- manage, a difficult question to assess from most datasets. Some evaluation research has utilized propensity scores based on analysis of predictors of program enrollment,4. In addition to reach, other program process measures assess the extent to which the self- management support interventions were implemented as intended. They essentially assess the program's performance of the self- management support processes called for in the program protocols. Examples of implementation measures reported in the program literature include the: Number of education sessions provided in person or by telephone. Frequency of coaching telephone calls. Duration of the telephone calls. Content of the telephone calls. An evaluation of a depression telecare program sponsored by an employee coalition, for example, reported that 1. They averaged 1. 1. An evaluation of a diabetes disease management program reported the number of educational mailings distributed, the number and average duration of telephone interactions, the average number of telephone interactions with individuals in the highest severity category, and the number of patients who could not be reached. Many studies assessed implementation by measuring the documentation of self- management support processes, such as patient education provided, action plan completed, patient goals collaboratively agreed on, smoking cessation counseled, referrals suggested, blood glucose self- monitoring training provided, and spacers and peak flow meters distributed. The asthma program described as a call- center model regularly reported to its sponsors the percentage of patients with a care plan, but the program did not use this measure in their study. Staff also may be surveyed about the performance of self- management support processes. One study. 53 reported the results of a survey that asked staff to report the frequency with which the following self- management support processes occurred: Support was promoted through problem- solving and empowerment methodologies. Patient self- management needs were assessed. Individualized written care plans were prepared. A written care plan was made available to primary care and urgent care staff. Spacer techniques were taught. Low- cost peak flow meters, spacers, and nebulizers (self- care tools) were made available to patients. Alternatively, similar measures may be based on reports by patients that they have received specified self- management support services. The DMAA Participant Satisfaction Survey, for example, includes items soliciting patients' reports of the frequency of different types of contact with program staff (such as receipt of educational materials, scheduled calls, and face- to- face meetings), which lifestyle changes have been emphasized by program staff (e. Inventory counts also may be used to measure the distribution of patient education materials or self- management support tools. Measures of Patient Self- efficacy and Knowledge. The principal goal of a self- management support program is to increase patients' knowledge and self- efficacy for self- management, and it makes sense to measure its ability to do so. Measures of self- efficacy assess people's confidence in their ability to perform or adhere to specific behaviors such as exercise, diet, or stress management or to overcome obstacles to the performance of these behaviors. Measures of knowledge and self- efficacy were used in some of the studies we reviewed. For example, when an independent delivery system implemented a 7- week, small- group self- management support program for patients with one or more chronic diseases, the evaluation included a self- efficacy measure of perceived adaptability to manage pain, fatigue, emotional distress, and other aspects of chronic illness. The patient's readiness to change also was assessed in a number of studies. Self- management, problem- solving skills, and self- management barriers were also assessed. It is important to identify measures that are validated, but it may be more difficult for this program area. More than one informant mentioned patient activation measures currently being developed and validated. According to interviewees, some programs use patient surveys to collect information on the patients' knowledge and self- efficacy. For several programs, routinely collecting such information is part of their program, e. What startup programs are necessary? I never install that software myself. Startup Programs Not needed? In Mexico, Chihuahua has the longest border, while Nuevo Le. New Mexico and Arizona each border two Mexican states (Chihuahua and Sonora; Sonora and Baja California, respectively). California borders only Baja California. Texas is more adjacent to Mexico than California. From west to east, below is a list of the border city . Guerrero, Chihuahua municipality. Fort Hancock, Texas . This period of sparse settlement included colonizers from different backgrounds. The area technically was part of the Kingdom of New Spain, but due to the lack of population and the diverse citizenry it had, it did not seem to belong to any country. This period lasted until the early 1. United States bought the lands known as the Louisiana Purchase from France and began to expand steadily westward in its pursuit of Manifest Destiny. El Paso and Juarez make up the second largest international metroplex after San Diego and Tijuana. The border itself was not clearly defined and remained so until the Mexican colony became independent from Spain and entered a period of political instability. Mexico attempted to create a buffer zone at the border that would prevent possible invasion from the North. The Mexican government encouraged thousands of their own citizens to settle in the region that is now known as Texas and even offered inexpensive land to settlers from the United States in exchange for populating the area. The influx of people did not provide the defense that Mexico had hoped for and instead Texasdeclared its independence in 1. That independence lasted until 1. United States annexed Texas. The constant conflicts in the Texas region in the mid 1. Mexican. In the terms of the peace treaty, Mexico lost more than 9. In addition, all disputes over Texas and the disputed territory between Rio Grande and Rio Nueces were abandoned. Five years later the Gadsden Purchase completed the creation of the current United States. The purchase was initially to accommodate a planned railway right- of- way. These purchases left approximately 3. Mexican nationals. Following the establishment of the current border a number of towns sprang up along this boundary and many of the Mexican citizens were given free land in the northern regions of Mexico in exchange for returning and repopulating the area. The Rio Grande shifted south between 1. By 1. 87. 3 the moving river- center border had cut off approximately 6. Mexican territory in the El Paso- Juarez area, in effect transferring the land to the United States. By a treaty negotiated in 1. Mexico regained most of this land in what became known as the Chamizal dispute and transferred 2. United States. Border treaties are jointly administered by the International Boundary and Water Commission (IBWC), which was established in 1. Once viewed as a model of international cooperation, in recent decades the IBWC has been heavily criticized as an institutional anachronism, by- passed by modern social, environmental and political issues. During the years of Mexican President Porfirio D. The mining industry also developed, as did the United States. By the early 2. 0th century companies from the United States controlled 8. Mexican economy overall, twenty- five percent of it in the border regions. The Revolution increased the political instability in Mexico, but did not significantly slow United States investment. It did reduce economic development within Mexico, however, and the border regions reflected this. As the infrastructure of communities on the United States side continued to improve, the Mexican side began to fall behind in the construction and maintenance of important transportation networks and systems necessary to municipal development. This means that the flow of illegal aliens is diverted into rural mountainous and desert areas, leading to several hundred migrant deaths along the Mexico. In 2. 00. 4 the updated strategy focused on command structures, intelligence and surveillance, enforcement and deployment of U. S. Border Patrol agents to better respond to threats at the border. The strategic planning led to broader policy development for the Department of Homeland Security which led to the Secure Border Initiative (SBI) in 2. U. S. The main components of SBI dealt with staffing concerns, removal capacity, surveillance and tactical infrastructure and interior enforcement. There was the allowance, historically, for voluntary returns of individuals apprehended at the border by Border Patrol agents. These voluntary returns, after the SBI of 2. The Immigration and Nationality Act permitted aliens to be formally removed with . The Department of Homeland Security has expanded between 2. Department of Homeland Security has also worked with the Department of Justice to increase the number of apprehended individuals crossing the border illegally who are charged with criminal offenses. Run a Command or Shell- Script Even after You Logout. When you execute a Unix job in the background ( using & , bg command), and logout from the session, your process will get killed. You can avoid this using several methods — executing the job with nohup, or making it as batch job using at, batch or cron command. It is quite simple to run a Unix command from Java. Runtime.getRuntime().exec(myCommand); But is it possible to run a Unix shell script from Java code? This quick tip is for beginners. If you’ve been using nohup for a while, leave us a comment and tell us under what situations you use nohup. In this quick tip, let us review how to make your process running even after you logout, using nohup. Nohup stands for no hang up, which can be executed as shown below. Nohup is very helpful when you have to execute a shell- script or command that take a long time to finish. In that case, you don’t want to be connected to the shell and waiting for the command to complete. Instead, execute it with nohup, exit the shell and continue with your other work. Explanation about nohup. When you execute a Unix job in the background ( using &, bg command), and logout from the session, your process will get killed. You can avoid this using several. Dear Folks, I wanted to run a particular shell program in a background, how do we do that. By default, the standard output will be redirected to nohup. So, your nohup. out will contain both standard output and error messages from the script that you’ve executed using nohup command. Instead of using nohup. Example: Printing lines to both standard output & standard error. System() and background procs w/ piping, redirs, ptys (Unix, Win32). Solutions for Running Windows on Mac OS X.
